Empowerment Care services – Baulkham Hills NSW

We are seeking a compassionate and dedicated Community Worker to join our team in Sydney NSW (Baulkham Hills, Botany, Roseville, Chester Hill) .

ECS provides high-quality, person-centered support to children and adults with disabilities living in their own homes or group settings. We’re committed to fostering independence, wellbeing and social inclusion through tailored care plans and community programs.

Your Role

As a Community Worker, you’ll deliver outreach and in-home support, helping clients build daily living skills, access education and vocational opportunities, and participate in social and recreational activities. You’ll work closely with health professionals, families and other agencies to ensure a holistic approach to each person’s goals.

Key Responsibilities

Conduct client assessments and co-design individual support plans

Provide practical assistance with daily living tasks (e.g., transport, shopping, appointments)

Facilitate community engagement through group activities and social inclusion events

Advocate on behalf of clients with external providers (health, education, housing)

Monitor and document progress, ensuring compliance with NDIS and ECS policies





Respond to client crises and implement appropriate interventions

Maintain a safe, respectful and empowering environment

What We’re Looking For

Minimum 2 years' experience as a community worker

Certificate III in Disability, Individual Support, Community Services, or equivalent (preferred)

Strong understanding of NDIS frameworks and relevant legislation

Excellent communication and interpersonal skills

Ability to work autonomously and collaboratively in a multidisciplinary team

Current NSW Working with Children Check and NDIS Worker Screening Clearance

Valid driver’s licence and reliable vehicle

Preferred Skills & Certifications

First Aid & current CPR certification (or willingness to obtain)

Experience in developing and reviewing support and behaviour plans

Cultural competency and ability to support clients from diverse backgrounds
