Our client is a community nursing and support service provider who has been servicing Western NSW for over 20 years. They are looking for a passionate Client Service (Care) Manager with a genuine interest in Aged Care to provide service to their clients living in their own homes in Dubbo and the Central West NSW.
As a Client Service (Care) Manager, you will be responsible for:
- Delivering person-centered co-ordination to clients
- Attending consultation visits.
- Completing initial intake assessments.
- Developing care plans in line with their budget.
- Case management and monitoring and reviewing client well-being
Requirements
- Current driversâ license and reliable vehicle
- Excellent communication skills
- Pride in the work you do with an eye for detail and high standard of care
- The ability to thrive in a close-knit team environment
