Our client is seeking a Clinical Care Manager that is responsible for managing a caseload of roughly 12-18 Child Safety and Disability Support clients while providing professional care management advice and support to the Principals and other members of the group to ensure that the design, implementation, management, and evaluation of professional care practices across assigned care facilities reflect recognised and relevant professional standards.

Responsibilities

Responsibilities include but are not limited to:

- Provide high-quality, professional care management advice and assistance to the Principals and the Executive Manager (Care Policy and Practice) particularly in relation to adults with an intellectual disability some of whom may have a dual diagnosis or be subject to forensic orders, children, and young people subject to Child Protection Orders arrangements and people who have a chronic illness.

- Provide qualified advice and support to the Executive Manager (Care Policy and Practice) and the Principals,



particularly when undertaking the development, write-up, implementation and ongoing management of Incident Reports, Individual Personal Plans (IPPs) for assigned clients with a disability and for children and young people under Child Protection Orders.

- Read and provide feedback to Behaviour Support Practitioners on accuracy of information, practicality, and implementation of Personal Behaviour Supports Plans (PBSPs).

- Represent the organisation at hearings of the Queensland Civil and Administrative Tribunal (QCAT) in matters associated with, for example, guardianship, financial administration and restrictive practices and other relevant client issues falling within the jurisdiction of the Tribunal.

- Develop in association with other health professionals, care treatment regimens for clients with medical, mental health and/or complex developmental and behavioural issues

- Conduct regular supervision and training of care staff on-site and prepare appropriate reports and documentation.

- Participate, on regular rotation,



in an ‘emergency after hours’ contact roster to provide professional advice to care staff in managing critical incidents affecting clients and client care.

Business Productivity Targets

Targets include but are not limited to:

- Provide high quality support, feedback and professional advice to key stakeholders.

- Write a variety of reports to maintain compliance within a variety of different governing bodies.

- Deliver household supervision between every 3 to 5 weeks.

Requirements

- Tertiary qualifications in Psychology, Nursing, Human Services, Social Work, Special Education, or similar fields. Postgraduate qualifications would be highly advantageous

- Demonstrated comprehensive knowledge of legislation, policy, and practice in a least one (1) of the following areas:

- Disability Care

- Forensic Disability

- Child Protection

- Aged Care

- Health Care

Possession of a

- Driver’s license

- Proof of Vaccination for COVID-19 in accordance with DCR policy

- Possession of or willingness to obtain:

- First Aid & CPR

- Blue Card

- NDIS Worker Screening Check

- LCS2 Background and History Check

- Willing to work on-site
