About the role The Clinical Nurse - Wound Management provides advanced clinical nursing expertise within the Post Acute Care Service (PACS), Community and Oral Health. Based at Caboolture Community Health Centre, the role delivers specialist wound management services and supports the provision of high-quality, evidence-based care for patients transitioning from acute care to community-based services.
The successful candidate will provide expert assessment, planning, implementation and evaluation of wound care interventions, while supporting consumers, families and healthcare professionals through education, leadership and clinical guidance. The role also contributes to quality improvement initiatives, professional development and the ongoing advancement of best-practice wound management services.
