Role Description The Complex Case Manager is a full-time, in office role based in Ballarat Central, VIC, responsible for coordinating and delivering comprehensive support to adults with disability who have complex care needs. Day-to-day tasks include developing person-centered support plans, conducting assessments, and monitoring progress toward individual goals. The role involves liaising with participants, families, and multidisciplinary teams, coordinating services with external agencies, and ensuring clear communication across all stakeholders.
The Complex Case
Manager maintains accurate documentation, manages risk, and supports compliance with relevant standards and policies.
This role also focuses on advocacy, crisis support where required, and continuous improvement of service quality to ensure participants receive the highest standard of care.
Qualifications
- Demonstrated skills in complex case management, including assessment, care planning, coordination of services, and ongoing review of participant needs.
- Strong communication and interpersonal skills, with the ability to build trust, maintain professional boundaries, and collaborate effectively with participants, families, and practitioners.
- Experience in disability support, community services, or allied health, with sound knowledge of person-centered practice and strengths-based approaches.
- Proficient organizational and time-management skills, including documentation, caseload management, and attention to detail in reporting and compliance.
- Ability to work autonomously on-site, exercise sound judgment in complex situations, and contribute positively to a values-driven team environment.
- Relevant tertiary qualification in Social Work, Psychology, Disability, Community Services, Nursing, or a related field (or equivalent experience).
- Understanding of relevant legislation, standards, and funding frameworks affecting disability services and complex case management.
- NDIS Screening Check, WWC.
