About the Service The Lives Lived Well Coffs Harbour Youth Residential Rehabilitation Centre is a specialist 24-hour alcohol and other drug residential service supporting young people aged 13-18 years who are involved with Youth Justice NSW.
The 5-bed residential program, including a dedicated withdrawal bed, provides a comprehensive 12-week rehabilitation framework. The service plays a vital role in helping young people address substance use and behaviours linked to offending, while building resilience, life skills and positive pathways for the future.
Your Prospect
We are currently seeking a Pre & Post Worker to join our team on an ongoing part-time basis (0.8 FTE, 30.4 hours per week).
In this varied and rewarding role,
you will:
- Conduct client assessments, screening and risk assessments.
- Support clients to prepare for and access residential treatment.
- Coordinate referrals, admissions and transitions into treatment.
- Develop care plans and continuing care arrangements.
- Facilitate individual and group-based support.
- Assist clients to build recovery supports within their local community.
- Develop and maintain referral pathways with key stakeholders.
This is a role for someone who enjoys building relationships, working autonomously and helping people achieve meaningful and lasting change.
About You
You will bring
- Experience in assessment, case management, care coordination or client support.
- Knowledge of evidence-based practice and person-centred approaches.
- Strong written and verbal communication skills.
- The ability to build effective relationships with clients and stakeholders.
- Excellent organisational skills and attention to detail.
- Confidence working independently and managing competing priorities.
- Knowledge of community services and referral pathways.
Relevant qualifications in Community Services, Counselling, Social Work, Psychology,
