About The Opportunity

This nurse-led service provides both Phase I (inpatient) and Phase II (outpatient) cardiac rehabilitation, delivering evidence-based education, support and rehabilitation to help patients recover, build confidence and transition successfully back into the community.

Working closely with cardiology, allied health and community services, you'll be part of a coordinated, patient-centred approach that supports patients throughout their cardiac recovery journey. You'll join a supportive and collaborative Cardiac Rehabilitation team, working alongside an experienced Cardiac Rehabilitation Nurse. You'll also receive ongoing clinical support from the Nurse Manager Critical Care Services, Cardiac Stream and Cardiology Clinical Nurse Consultant (CNC).

A structured orientation and on-the-job training program will support you to develop your knowledge of the local Cardiac Rehabilitation Model of Care, outpatient program delivery, referral pathways and service processes.

What You Will Be Doing

As a Cardiac Rehabilitation Nurse, you'll provide high-quality nursing care across inpatient and outpatient settings, supporting patients following cardiovascular events and procedures.

Your Responsibilities Will Include

- Providing pre-discharge cardiac education and support.
- Coordinating referrals to outpatient cardiac rehabilitation services.
- Conducting comprehensive cardiac rehabilitation assessments.
- Facilitating cardiac education and exercise programs.
- Monitoring patient progress and supporting secondary prevention strategies.
- Coordinating and scheduling patient appointments and rehabilitation programs.
- Maintaining accurate clinical documentation and patient records.
- Liaising with cardiology, allied health, primary care and community providers.
- Supporting patients to develop the knowledge, confidence and skills required to manage their cardiac health.
- Contributing to service development, quality improvement and evidence-based practice.

The position reports to the Nurse Manager Critical Care Services and offers the opportunity to work both independently and collaboratively within a multidisciplinary cardiac care team.

About You

We're looking for a passionate Registered Nurse with strong cardiac knowledge and a genuine interest in supporting patients through recovery and secondary prevention.

You'll bring excellent communication and patient education skills, allowing you to confidently support patients to understand and manage their cardiac condition.



You'll be highly organised, with strong time management skills and the ability to coordinate referrals, appointments and outpatient programs effectively. You will be confident working autonomously while also contributing positively to a multidisciplinary team and building strong relationships with cardiology, allied health, primary care providers and community services.

Sound clinical judgement, initiative, a patient-centred approach and a commitment to ongoing learning and quality improvement will be key to your success.

Essential Requirements

- Current registration as a Registered Nurse with AHPRA
- Demonstrated cardiac nursing knowledge and experience.
- Relevant NSW Health mandatory requirements.

Desirable

- Postgraduate qualification in Critical Care, Cardiac Nursing or equivalent.
- Previous experience in cardiac rehabilitation or cardiovascular nursing.
