We are seeking experienced Physiotherapists to deliver comprehensive face-to-face aged care assessments across Perth, WA. 10 positions available across the metro area:
- Metro Perth - 5 positions
- North & East Perth - 3 positions
- South Perth - 2 positions
In this face-to-face role, you will conduct comprehensive assessments to determine the care and support needs of older Australians. Working as part of a multidisciplinary team, you will undertake holistic assessments considering clients' health, functional capacity, social circumstances and overall wellbeing, helping them access appropriate aged care services and supports.
- Conduct comprehensive face-to-face aged care assessments in accordance with relevant assessment frameworks and guidelines
- Assess clients' clinical, functional, cognitive, psychosocial and environmental needs
- Develop recommendations and support plans to facilitate access to appropriate aged care services
- Complete detailed assessment documentation and reports
- Work collaboratively with multidisciplinary clinicians and stakeholders
Benefits:
- $85,000 - $105,000 per annum + superannuation

About You:
- Relevant degree in Physiotherapy
- Current AHPRA registration
- Strong communication and clinical assessment skills
- Experience in aged care, community health or rehabilitation highly regarded
- Ability to work autonomously in the community
- Excellent documentation and stakeholder communication skills
- Current driver's licence and own vehicle
- Eligible to work in Australia
