Connective Service's continues to grow and we are again looking for an experienced Support Coordinator (Level 2 and/or Specialist) who would like to work alongside some great team members!

Founded in 2024, Connective Services was created with a clear purpose to deliver high-quality, person-centred support coordination that genuinely improves outcomes for participants. The work is grounded in transparency, accountability, and a strong commitment to doing things properly. With extensive hands-on experience in the disability sector and a strong focus on advocacy for inclusive care, the service reflects a deep understanding of the challenges within the system and a commitment to improving it. This approach has been recognised through the 2025 City of Joondalup Community Citizen of the Year award.
As a small business, I’m intentionally building a small, trusted network of coordinators who align with this approach, not a high-volume model.
The OpportunityContractor role (ABN required)Part-time,



with the capacity to grow into full-timeFlexible and autonomous working arrangementsSupporting participants across Perth metro, including complex needsBased in Perth's Southern Suburbs - WESTERN AUSTRALIA
About the RoleYou’ll be working closely with participants and their support networks to:Deliver tailored, person-centred support coordinationManage and maintain case files and documentationCoordinate and monitor service deliveryMaintain strong communication with all stakeholdersEnsure compliance with NDIS standards and best practice
This role would suit someone who:Has experience supporting complex or high-risk participants, particularly aging participants diagnosed with degenerative neurological conditions.Demonstrates solid interpersonal and communication skillsHas solid experience in case management and coordinationIs confident working both independently and collaborativelyTakes a quality-focused, ethical approach to their workHas a strong, practical understanding of the NDIS
Relevant qualifications in Community Services, Social Work, or similar are highly regarded, as well as valid WWC, NDIS worker screening and police checks.
