If you enjoy helping people navigate the NDIS, coordinating quality supports and empowering participants to achieve their goals, we would love to hear from you.About the RoleThis is more than coordination, it is about walking alongside people and helping them navigate systems with confidence.You will support participants with complex needs to access services, understand their NDIS plans, and build the skills to manage their supports over time.
Your work will help remove barriers, strengthen outcomes and create pathways to independence.How you will make a difference:Coordinating personalised supports for participants and familiesSupporting access to the NDIS, including eligibility and plan implementationBuilding strong relationships with participants, community and service providersAdvocating for participants and helping them achieve their goalsWorking collaboratively across health,
disability and community servicesIdentifying service gaps and helping improve access pathways for communityYou are someone who leads with respect, builds trust easily, and is committed to positive outcomes for community.You will bring:Experience in NDIS support coordination or working with people with disabilityStrong ability to support individuals with complex needs and their familiesKnowledge of NDIS guidelines, legislation and community servicesA strengths-based, culturally responsive approachConfidence working with a wide range of stakeholdersPassion for empowering Aboriginal and Torres Strait Islander peopleCurrent driver's licence (and relevant clearances)This is an Aboriginal and/or Torres Strait Islander Identified Position.
