APM in Townsville region is seeking an Aged Care Comprehensive Assessor to identify the needs of older adults and connect them with services that help them live independently with the highest quality of life across the Townsville (Northern) Region.
Daily travel is a core requirement of this role, traveling throughout the service area to meet client needs and provide support where needed most. You will work within a multidisciplinary team and engage with families and carers as key stakeholders.
