Mrs Chrissy is seeking an experienced and compassionate NDIS Support Coordinator to join our team at our Revesby, NSW office. This is a full-time position offering a meaningful opportunity to make a real difference in the lives of people with disabilities. As a Support Coordinator, you will be responsible for coordinating and managing support services for NDIS participants, ensuring they receive appropriate and person-centred care that aligns with their goals and aspirations.
Key responsibilities
- Assess the needs and goals of NDIS participants through comprehensive consultations and documentation
- Develop and maintain individualised support plans that reflect participant objectives and preferences
- Coordinate the delivery of support services from appropriate providers, ensuring quality and consistency
- Monitor participant progress against goals and adapt support plans as required
- Liaise between participants, their families, support providers and other stakeholders to ensure effective communication and service delivery
- Manage participant budgets and financial arrangements in accordance with NDIS guidelines
- Complete all necessary documentation, reporting and compliance requirements in accordance with NDIS legislation and regulations
- Provide ongoing support and advocacy to participants to maximise their independence and social participation
- Conduct regular participant reviews to evaluate the effectiveness of support arrangements
- Maintain accurate and confidential case files and records
What we're looking for
- Qualifications relevant to community services, disability support, social work or allied health
- Proven experience working as a Support Coordinator within the NDIS environment, or equivalent experience in disability support services
- Robust knowledge of NDIS policies, legislation, and participant-centred approaches
- Excellent communication and interpersonal skills with the ability to build rapport with diverse individuals
- Demonstrated ability to manage multiple cases and priorities effectively whilst maintaining attention to detail
- Strong problem-solving skills and ability to think creatively about solutions
- Current knowledge of local support services, providers and community resources
- Proficiency with relevant IT systems and databases, including NDIS IT platforms
- Current Working with Children Check and/or Working with Vulnerable People clearance, or willingness to obtain
- A person-centred, empathetic approach to service delivery with a commitment to promoting participant choice and control
- Valid Australian driver's licence and reliable transport
