Outreach Program Coordinator

Duration: 12-month contract, possible 12-month extension
Location: Melbourne
Hourly Rate: $67.24 - $76.36
Clearance: Ability to obtain and maintain baseline

Mandatory requirements: Qualifications as a registered Psychologist / Occupational Therapist / Registered Nurse / Social Worker / A Masters-level counselling qualification with full relevant registration.

About the Role

This role coordinates and supports the delivery of community-based health and wellbeing programs, working with external providers and stakeholders to ensure high-quality, client-focused services. The position is responsible for program coordination, provider engagement, quality assurance, stakeholder management, and continuous improvement initiatives. Solid communication, problem-solving, and relationship management skills are essential, along with the ability to manage complex issues while maintaining compliance with organisational policies and privacy requirements.

Key Responsibilities
Coordinate and support external service providers and stakeholder relationships.
Manage provider onboarding, support, and performance monitoring.
Review referrals and assess service suitability.
Maintain program records, databases,



and provider availability information.
Coordinate the delivery and promotion of programs, workshops, and group activities.
Respond to and help resolve complaints, concerns, and complex service issues.
Act as a key point of contact and escalation for operational matters.
Contribute to quality assurance, program evaluation, and continuous improvement activities.
Participate in career development, supervision, and community engagement initiatives.
Support organisational projects, policy development, and strategic objectives.
