District 360 is a registered NDIS provider growing our Western Australia team. We are looking for experienced Support Coordinators to join us on a casual basis.
What you will be doing
Help participants understand and implement their NDIS plans
Source and coordinate services from community, mainstream, and funded providers
Monitor plan budgets and ensure supports align with participant goals
Build robust relationships with participants, families, and support networks
Complete accurate case notes, service agreements, and progress reports
Work within NDIS Practice Standards and the NDIS Code of Conduct
What you need
Certificate IV in Disability, Community Services, or related field (degree preferred)
Demonstrated NDIS Support Coordination experience
Strong knowledge of plan budgets, support categories,
and the participant portal
Current valid driver's licence and reliable vehicle
Current NDIS Worker Screening Check (WA) or willingness to obtain prior to start
Current First Aid and CPR certificate
