This role involves coordinating support services for individuals with disabilities, ensuring their well-being and inclusion in the community.
Responsibilities include developing support plans, liaising with service providers, and advocating for clients.
The ideal candidate will have a background in disability services, excellent communication skills, and a passion for making a difference in the lives of others.

Work-life balance Task & responsibilities
Conduct intake interviews and comprehensive assessments of clients' social, emotional, financial, and housing needs.
Develop support plans for individuals with disabilities
Liaise with service providers to coordinate support services
Advocate for clients to ensure their needs are met
Monitor and evaluate the effectiveness of support services




Provide guidance and support to clients and their families
Qualification & experience
Relevant in disability services or related field
Minimum 1 year Experience working with individuals with disabilities
Excellent communication and interpersonal skills
Ability to work effectively in a team
Understanding of disability rights and advocacy
Hold a current, unrestricted driver's licence.
Ensure you have up-to-date immunisations as required Completed certifications and checks such as a
First Aid Certificate, Working with Children Check, Police Check and/or NDIS Worker Screening Check
