We believe everyone deserves the opportunity to thrive, build confidence, and participate fully in their community.If you're compassionate, reliable, and committed to making a genuine difference, we'd love to welcome you to our team.About the RoleAs a Lifestyle Facilitator (Support Worker), you will provide tailored supports that promote independence, community inclusion, and improved quality of life for people living with disability.You'll work alongside Participants to build confidence, develop life skills, and achieve the goals outlined in their NDIS plans.Your responsibilities will include:Supporting Participants with daily living activities and personal careEncouraging independence, choice, and decision-makingAssisting with social, recreational, educational, and employment-related activitiesFacilitating meaningful community participationImplementing Positive Behaviour Support strategies where requiredSupporting Participants with complex needs in a respectful and person-centred mannerAssisting with household tasks including meal preparation, cleaning, and laundryPerforming safe manual handling tasks, including hoist transfers and wheelchair assistanceWorking collaboratively with families, allied health professionals, and other support networksMaintaining accurate documentation and complying with all abbaCare policies, NDIS Practice Standards, and legislative requirementsWhy Join abbaCare?

and qualified approachAbility to advocate for Participants while promoting independenceExcellent communication and interpersonal skillsExperience supporting people with disability and/or mental health conditionsExperience supporting Participants with complex behaviours and support needsCompetency in manual handling techniquesExperience working active overnight shiftsAbility to work independently and as part of a multidisciplinary teamMandatory RequirementsApplicants must hold or be willing to obtain:Queensland Green P-Plate or Open Driver LicenceReliable, fully comprehensively insured vehicleCurrent Blue Card (Working with Children Check)NDIS Worker Screening Clearance (Yellow Card)Completion of the NDIS Worker Orientation ModuleCurrent First Aid and CPR CertificateEvidence of COVID-19 vaccination (minimum two doses, where required)Desirable Skills and ExperienceWhile not essential, the following will be highly regarded:Experience working within Supported Independent Living (SIL)Knowledge of Positive Behaviour Support PlansUnderstanding of privacy, confidentiality, and safeguarding principlesExperience building collaborative relationships with external service providersDiversity and InclusionAt abbaCare, diversity makes us stronger.
