Our growing Support at Home program helps older people live safely and independently in their own homes.
Services are tailored to individual needs and may include clinical care, allied health, personal support and assistance with daily living.
About the role
Working under the guidance of Allied Health Professionals, you'll deliver meaningful activities, reablement support, prescribed exercise and functional programs tailored to each client's goals.
In addition, you'll provide hands-on Support Worker services in clients' homes and the community.
This includes personal care, assistance with activities of daily living, domestic support, social support, community access and transport, delivered in accordance with each client's care plan.
By combining reablement-focused interventions with practical day-to-day support, you'll build meaningful relationships with clients while helping them remain safe, independent and connected to their community.
This role is available on a casual basis or part-time basis, working 2 days per week.
About you
You embody Catholic Homes values of love, hospitality, excellence and joy, and bring compassion, patience and a person-centred approach to supporting older people with daily living activities, promoting dignity, choice and independence in every interaction.
In addition, you will have:
Certificate III or IV in Leisure and Lifestyle, Allied Health Assistance or similar
Experience as a Lifestyle Facilitator,



Physiotherapy and/or Occupational Therapy Assistant in a community or home care setting
Experience as a Support Worker, or a willingness to develop skills in providing personal care and in-home support services
Current C class drivers' licence
A roadworthy, registered vehicle with comprehensive car insurance
Experience creating and implementing person centred activities for aged care clients
Comfortable and confident driving between clients homes
A strong commitment to reliability, flexibility, and professionalism
Excellent communication and interpersonal skills
Intermediate computer skills
Be a team player with the ability to multitask and prioritise duties, and the ability to work independently
Willingness to work across different sites as required.
