About the team
Peter Mac @ Home (PM@H) is the acute home-based oncology service for Peter Mac patients/clients and is available 7 days per week, 365 days a year.
The service incorporates Hospital in the Home (HITH) and Home-based cancer care as funded through the DHHS.
About the role
The PM@H unit is currently seeking an experienced Registered Nurse to join our team as a community nurse.
This role involves nurses visiting patients in their homes.
A commitment to providing excellent community care, professional development of self and others and a commitment to providing evidence-based care is an essential requirement of the role.
We are looking for someone who can confidently work autonomously and within a team setting and will join us in contributing to theinnovation of the service.
Roles and Responsibilities include:
Central Venous Access Device Management (CVAD)
V fluid administration
Antibiotic Administration
Drain Tube Management/VAC Management
Disconnection of IV Chemo Baxter
Administration of Systemic Anti-Cancer Therapy (Chemotherapy and Immunotherapy)
Reactionary area care post radiation therapy
About you
You are an experienced Registered Nurse with a minimum of three years post-graduate experience and a background in acute care, with experience in medical oncology, haematology, surgical nursing or a related clinical setting.
You have demonstrated skills in the administration of Systemic Anti-Cancer Therapy (SACT), IV cannulation and CVAD management, with a strong commitment to evidence-based, patient-centred care.
Confident working both autonomously and as part of a multidisciplinary team, you bring excellent clinical judgement, communication skills and a passion for delivering high-quality care in the community setting.
A current and valid driver's licence is essential for this role.

Excellent communication skills and proven ability to work effectively in a multidisciplinary team

Key Accountabilities
1. Management of Patient Care Continuum
Responsibility for systematically assessing the health care needs of individuals and for the formulation of a nursing plan of care, its implementation and its evaluation
Assesses patients in a systemic way, identifies the patientsandrsquo health problems and validates nursing plans of care by involving the patient and/or family in the plan of care.
Develops appropriate teaching plans for the patient/family
Integrates information from the nursing assessment and from other health care team members
Revise the nursing plan of care appropriately to reflect current or changing status of the patient




Adheres to the principles of defensible documentation and ensures that nursing documentation is complete, up to date, signed and dated
Acts as a patient/carer advocate in relation to the patient/carer rights to all aspects of care, knowledge and privacy
Management of patients including patient education and discharge planning
Patient records show documented assessments and plan of care and continuous evaluation of responses to treatment
Maintains professional skills and updates knowledge by attending frequent professional forums and education

Contributes to creating a working environment conductive to learning and professional development
