Adelaide, South Australia | Temporary Part Time – 0.4 FTE | 2-Year Contract
Street Side Medics is a not-for-profit mobile medical service led by GPs and other health professionals, delivering primary healthcare to vulnerable populations and people experiencing homelessness.
Working in partnership with established homelessness charities, shelters and support services, our aim is to improve access to healthcare and achieve better health and wellbeing outcomes for people who are vulnerable or without stable housing.
About the role
We are seeking a
Clinical Nurse Lead
to oversee the clinical coordination and delivery of Street Side Medics' Primary Care Outreach service across the Adelaide region.
Working closely with our Clinical Manager, SA Service Coordinator and volunteer workforce, you will provide hands-on clinical leadership at outreach clinics and help ensure safe, effective and continuous care for our patients.
This position has a robust focus on clinical support, coordination of care, patient follow-up and operational delivery of clinics.
Key responsibilities
You will:
Act as a senior clinical presence at mobile clinics, supporting safe, efficient and high-quality care delivery.
Coordinate patient follow-up, including results, referrals and ongoing patient management between clinics.
Work with service partners to develop care pathways linking outreach healthcare with local health and community services.
Support the onboarding and mentoring of clinical volunteers.




Facilitate pre- and post-clinic huddles and oversee clinic safety and smooth clinical operations.
Foster a positive, collaborative and well-supported team environment.
Participate in quality improvement, health promotion and clinical governance activities.
Assist with data collection, patient demographic monitoring and service evaluation.
About you
To succeed in this role, you will be:
An AHPRA Registered Nurse with a current South Australian driver's licence.
Ideally experienced for 3+ years in community health, outreach nursing or a similar setting.
Experienced in clinical coordination and patient follow-up.
Confident working collaboratively within multidisciplinary teams.
Experienced in supporting or working with vulnerable populations, including people experiencing homelessness.
An excellent communicator who can build trust with patients, volunteers and external stakeholders.
Highly organised and able to work independently.
Adaptable and resourceful in a mobile or outreach healthcare environment.
Experience with vaccination services or cold-chain management would be highly regarded but is not essential.

This is a
temporary 0.4 FTE position for a two-year contract, commencing October ****
.
The role is dependent on ongoing funding and may have the opportunity to continue or increase in hours as the service expands.
Some regular after-hours work will be required, currently expected to include approximately
one early-evening clinic per week
