About the Role:

We are seeking experienced and compassionate Support Worker to join our growing team on a permanent full-time basis.
Key Responsibilities
Provide personal care, including assistance with showering, dressing, grooming, toileting and mobility.
Assist clients with domestic assistance, including cleaning, laundry and maintaining a safe home environment.
Prepare meals and assist with nutrition in accordance with individual dietary requirements.
Provide companionship, social support and assistance with community participation.
Transport clients to medical appointments, shopping and social activities where required.
Monitor clients' wellbeing and promptly report any changes in health or care needs.
Maintain accurate records, progress notes and documentation in accordance with organisational policies.
Follow individual care plans and comply with all aged care standards, workplace health and safety requirements and infection control procedures.
Work collaboratively with coordinators, nurses,



allied health professionals and other members of the care team to achieve positive outcomes for clients.
Essential Requirements
Certificate III in Individual Support (Ageing and Disability), or an equivalent relevant qualification.
At least one year of recent highly relevant experience.
Demonstrated ability to provide compassionate, respectful and person-centred care.
Excellent communication and interpersonal skills.
Current National Police Clearance.
Current First Aid and CPR Certificate (or willingness to obtain).
Current Australian Driver Licence and access to a reliable, insured motor vehicle.
Current Influenza vaccination and other vaccinations as required.
