Join Western District Health Service as a Support at Home – Case Manager , where you'll deliver high-quality, person-centred care management and make a meaningful impact in your community every day.
This is a role for someone who thrives on building relationships, problem-solving, and coordinating supports that enhance quality of life, dignity, and choice for older people.
What You'll Be Doing Person-Centred Care Deliver tailored care management that supports independence, dignity, and individual choice for older people living at home.
Care Planning Coordination Develop, implement, and review personalised care plans in collaboration with participants, families, and service providers.
Stakeholder Engagement Work closely with clients, families, healthcare professionals, and external providers to ensure coordinated, seamless care.
Budget Program Management Manage individual program budgets, ensuring supports are delivered effectively within funding guidelines.
Monitoring Review Regularly assess client outcomes and adjust care plans to meet changing needs.
Documentation Compliance Maintain accurate records and ensure compliance with program requirements, standards, and reporting obligations.
Teamwork Collaboration Work independently and as part of a multidisciplinary team within a dynamic and supportive environment.
What You Bring • Experience in aged care case management • Demonstrated ability to manage budgets and funding programs • Strong communication and relationship-building skills • Ability to coordinate complex supports in a community setting • Excellent organisational and time management skills • Commitment to person-centred care and positive client outcomes • Ability to work both independently and within a multidisciplinary team Why Join Western District Health Service?
