We're looking for an experienced and compassionate Support Coordinator who is committed to achieving positive outcomes for participants and their families.

About the Role

This is a dedicated Support Coordination role focused on helping participants understand, implement and maximise the benefits of their NDIS plans. The role is centred on delivering high-quality, participant-focused support, building robust relationships, and assisting participants to achieve their goals and access the services they need.

What You'll Be Doing

- Supporting participants to understand and implement their NDIS plans
- Coordinating services and connecting participants with suitable providers
- Building strong relationships with participants, families, and stakeholders
- Promoting independence, choice, and capacity building
- Monitoring budgets and ensuring NDIS compliance
- Preparing reports and supporting plan reviews




- Advocating for participant goals and positive outcomes

What You'll Bring

- Experience in NDIS Support Coordination and/or case management
- Strong understanding of the NDIS and relevant frameworks
- Excellent communication and relationship-building skills
- Strong organisational and time management abilities
- Qualifications in Disability, Social Work, Psychology, Allied Health, or a related field (desirable)
- Confidence using Microsoft Office and case management systems

Essential Requirements

- NDIS Worker Screening Check
- Working With Children Check (WWCC)
- National Police Check
- Current Driver's Licence and reliable vehicle
