About the opportunity
As Care Finder, you will play a meaningful role insupporting senior Australians who need intensive assistance to understand,navigate and access aged care services. You will build trusted relationships, connectpeople with practical community supports, and help them make informed choicesthat promote independence, dignity and wellbeing.
This full-time role also offers working-from-homeflexibility, supporting a balanced and responsive way of working.
In this role, you will apply a person-centred approach that upholdsdignity, respect, choice and self-determination, while working in line withCare Finder policy and guidelines.
Your focus will include:
- Undertaking holistic assessments and supportingplanning to determine and document individual needs and access to supports.
- Ensuring that individuals are valued andrespected and that supports aim to promote independence and clientself-determination.
- Visiting clients in home, engaging clients indifferent environments, and informational outreach to service providers and thecommunity.
- Meeting with providers to arrange services andconnecting with other relevant supports in the community.
- Organising and participating in case conferencesand interagency meetings in a collaborative and consultative manner.
- Promoting Footprints programs and services andadvocating for service options for the target group, in the wider community,through sound working relationships with partners and networks withingovernment, non-government and private sectors.
- Undertaking administrative functions, includingmaintenance of client data base, maintaining a high standard of client files,reporting requirements, and data input activities.
What you will bring:
- Experience working with a diverse client groupincluding older people, people with disabilities, people experiencing mentalillness, and people experiencing homelessness.
- Demonstrated experience in community networkingand working collaboratively with external service providers to plan and provideconsistent, responsive, and appropriate client focused services.
- Local community connections with the Care Findertarget population or a specific sub-group within the care finder targetpopulation.
- Ability to attain a detailed understanding ofthe range of aged care supports and services available in the local community,the process to access, and any eligibility requirements for other relevantsupports.
- Computer literacy and the ability to effectivelymanage a client data base.
Requirements:
- Relevanttertiary qualification and/or relevant experience.
- CurrentQLD Driver's Licence and access to registered vehicle.
- NDISWorker Screening (Yellow Card) and Blue Card.
- CurrentFirst Aid and CPR Certificate.
