About the Role | Tasks & Responsibilities
As a Support Worker at Co-Care, you will play an important role in supporting individuals to live independently, build confidence and actively participate in their community.
Your responsibilities will include:
- Providing person-centred support tailored to each client’s individual needs, preferences and goals.
- Assisting clients to maintain their accommodation and develop skills to manage their home setting.
- Supporting clients to develop independent living and self-management skills.
- Promoting community inclusion and supporting clients to participate in meaningful activities and community life.
- Working collaboratively with clients to identify and work towards achieving their personal goals.
- Providing support across a flexible 7-day roster, including mornings, evenings and weekends, on a casual basis.
- Liaising with service providers and other relevant stakeholders to ensure clients receive appropriate support, referrals and information.
- Maintaining a respectful, professional and empowering approach in all interactions with clients and their support networks.
Essential Requirements
- A current Australian Driver’s Licence and access to a reliable motor vehicle.
- A current National Police Check and Working with Children Check (WWCC).
- A current NDIS Worker Screening Check.
- A current First Aid Certificate.
- Strong professional boundaries and the ability to follow policies, procedures and directions.
- Excellent communication, reliability and a genuine commitment to providing person-centred support.
Highly Desirable
- Previous experience supporting clients with complex needs and behaviours of concern.
- Experience working with clients who may demonstrate challenging behaviours.
- A calm, respectful and professional approach when responding to complex or challenging situations.
