Are you an experienced Support Coordinator with an established participant and referral network?

Ableconnect is a growing NDIS provider based in Fitzroy, Melbourne, delivering person-centred supports that empower people with disability to achieve their goals and live fulfilling lives.

We are seeking an experienced Level 2 and/or Level 3 Support Coordinator to join our growing team on a full-time subcontractor basis.

This opportunity is particularly suited to a Support Coordinator who has existing professional networks, referral relationships and the ability to build and maintain their own participant caseload.

About the Role

Based at our Fitzroy Head Office, you will manage your own participant caseload and deliver high-quality Level 2 and/or Level 3 Support Coordination in accordance with NDIS Practice Standards.

You will have the opportunity to:

- Build and manage your own participant caseload.
- Deliver high-quality Level 2 and/or Level 3 Support Coordination.
- Develop strong, professional relationships with participants, families, guardians and stakeholders.
- Support participants to understand and implement their NDIS plans.
- Coordinate services and connect participants with appropriate providers.
- Develop person-centred strategies to support participant goals.
- Build and maintain referral and professional networks.




- Identify opportunities to introduce suitable participants to Ableconnect's Support Coordination, Supported Independent Living (SIL) and Community Participation (CP) services.
- Work closely with our Service Delivery team while maintaining appropriate qualified boundaries and managing conflicts of interest.
- Maintain accurate case notes, reports, documentation and billing.

We Are Looking for Someone Who:

- Has a minimum of 2 years' experience as a Support Coordinator.
- Has a strong understanding of the NDIS and person-centred practice.
- Has an established network across the disability, community, health or allied health sectors.
- Has experience developing referral relationships and building participant caseloads.
- Is confident engaging with participants, families, guardians and professionals.
- Is proactive, motivated and commercially minded while maintaining a strong commitment to participant outcomes.
- Can work autonomously and manage a caseload effectively.
- Has excellent communication, organisation and documentation skills.
- Is committed to high-quality service delivery and the NDIS Code of Conduct.

This role is particularly suited to an experienced Support Coordinator who can bring established professional relationships and referral opportunities.





We are looking for someone who understands the local NDIS landscape and has the ability to develop a sustainable caseload through existing networks, skilled relationships and new referral opportunities.

There is also an opportunity to contribute to the growth of Ableconnect's broader services, including SIL and Community Participation, where appropriate and in accordance with NDIS requirements and conflict-of-interest obligations.

You must have:

- Australian Business Number (ABN).
- GST registration, where applicable.
- NDIS Worker Screening Check.
- National Police Check with 12 months.
- Current Victorian Driver Licence.
- Reliable vehicle and comprehensive motor vehicle insurance.
- Current First Aid and CPR Certificate.
- NDIS Worker Orientation Module Certificate.
- Supporting Safe and Enjoyable Meals Certificate.
