About the opportunity
As part of a multidisciplinary team, the Registered Nursewill undertake comprehensive clinical assessments, development ofnursing/health support plans in partnership with the client and/orrepresentatives while collaborating with internal and external stakeholders tomeet individual health and wellbeing.
You will:
- Provide nursing expertise and mentoring toRegistered Nurses, Enrolled Nurses, coordination staff and support workerstaff.
- Provide leadership and role modelling todevelop, implement and evaluate staff and/or client development programs tosupport the provision of Aged Care Services.
- Coordinate the maintenance of standards ofnursing care within the Aged Care cohort of clients.
- Act as a consultant on request in the area ofstaff proficiency; for the purpose of facilitating the provision of qualitynursing care.
- Deliver direct and comprehensive nursing care toAged Care clients with complex nursing care needs.
- Undertake comprehensive clinical assessments anddevelop nursing and health support plans with a focus on wellness andrestorative practices.
- Complete specialised assessments in accordancewith funding guidelines to support clients with complex needs e.g., continenceadvisory service.
- Collaboratively operate as a member of amulti-disciplinary team to refer and advocate for appropriate specialisedhealthcare services for clients.
- Work within Footprints Community Care andClinical Governance Framework.
- Assist in the ongoing development and evaluationof program practice, ensuring continuous quality improvement and compliancewith the Aged Care Quality Standards and the Human Services Quality Standards.
What you will bring:
- Experience in Aged Care, preferably in acommunity care setting.
- Experience in Palliative Care and/or MentalHealth Care (desirable).
- Demonstrated experience in health assessmentsand community care, with demonstrated knowledge and understanding of theassessment of complex care, chronic health care, and psychosocial needs.
- Demonstrated knowledge and experience incommunity networking in collaboration with other service providers to plan andprovide consistent, flexible, and appropriate service responses.
- Demonstrated understanding and commitment toquality improvement processes, including Occupational Health and Safety andrisk management.
- Excellent written and verbal communicationskills, and effective time management skills.
Requirements:
- Registered Nurse,
current registration withAHPRA.
- Current Queensland Class C Driver’s Licence.
- NDIS Worker Screening Check or current YellowCard and Blue Card.
- Current First Aid and CPR Certificate.
