Heart Worx Tamworth NSW is seeking a Support Coordinator who can also work as a Psychosocial Recovery Coach. This hybrid role offers permanent full time or part-time employment and supports participants to understand and use NDIS plans, coordinating providers and ensuring follow-through.

No prior dual-role experience is required; the successful candidate will have strong writing and communication skills, good judgement and the ability to manage a busy caseload with professional boundaries.
