Visa2land Pty Ltd This role is for a compassionate and dedicated Community Worker to support the coordination of our internal services and contribute to our Community Connections Program. In this meaningful role, you will help ensure individuals living with ageing and disabilities receive the right support to meet their current and future needs, including assistance to access NDIS and Aged Care Plans.

Task & responsibilities

- Assess participants’ needs, goals and circumstances and develop appropriate support programs.
- Provide compassionate, person-centred support to people with disabilities and older people.
- Coordinate, implement and monitor NDIS and Aged Care support plans.
- Meet with participants to assess their needs, discuss progress and identify appropriate services.
- Refer participants to relevant community, health, disability and support agencies where required.
- Monitor and report on participant progress and maintain accurate records.




- Evaluate support programs and adjust services to meet changing participant needs.
- Work collaboratively with families, carers and service providers to ensure coordinated support.
- Ensure services comply with organisational policies, NDIS and Aged Care requirements and relevant legislation.

Qualification & experience

- Minimum 1 year experience in the field highly desirable.
- Minimum - a diploma in community services or related field
- Demonstrate qualities of empathy, compassion, and integrity.
