The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

Are you an experienced Social Worker passionate about making a meaningful difference in the lives of children and young people? An exciting opportunity exists to join the Out-Of-Home Care Health Pathway Program (OOHC HPP), where you will play a pivotal role in supporting the health, wellbeing and recovery of infants, children and young people living in Out-Of-Home Care.

As a Social Worker Level 3, you will deliver specialist trauma-informed assessment, intervention and consultation services for children and young people aged 0–18 years. Working within a multidisciplinary team, you will contribute to comprehensive health assessments and ongoing health management to promote, protect and maintain the physical, developmental, psychosocial, sexual and mental health of children and young people in care.

In this role, you will undertake comprehensive psychosocial assessments and contribute to the development, implementation and evaluation of Health Management Plans.



You will provide targeted therapeutic and psychosocial interventions that address the impacts of trauma, adversity and disrupted attachments, ensuring care is responsive to each child’s developmental needs, placement circumstances and individual goals.

You will also provide specialist advice and consultation to carers, caseworkers and health professionals, supporting the delivery of coordinated and integrated care. Through education, group facilitation and capacity-building activities, you will help strengthen the skills and knowledge of those working with children and young people in Out-Of-Home Care, ultimately contributing to improved health and wellbeing outcomes.

What you will bring to the role

1. Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers, including the ability to demonstrate a practical application of the AASW Code of Ethics.
2. Demonstrated post graduate experience providing advanced, trauma informed psychosocial assessment and intervention to children and young people (0-18 years) and families with complex needs, including those impacted by trauma, abuse, neglect, developmental trauma, disability and/or complex psychosocial circumstances.
3. Demonstrated knowledge of Child Protection Legislation and policy and procedures, with a focus on Out-Of-Care.
4. Demonstrated advanced knowledge of the health, developmental and psychosocial needs of children and young people who have a high level of vulnerabilities, and the ability to ensure the voice of the child or young person is central to assessment, care planning and intervention.
5. Demonstrated ability to contribute to, review and coordinate care (HMP’s) including making clinical recommendations, coordinating referrals and navigating complex service systems.
6. Ability to create and maintain close working relationships with NSW Health Services, Dept of Communities and Justice and Non-Government Organisations and multidisciplinary colleagues.
7. Demonstrated ability to prioritise work and manage stressful situations; ability to work autonomously and exercise sound professional judgement in complex or unfamiliar situations.
8. Demonstrated ability to initiate, lead, complete and share quality improvement activities and service evaluation processes with other staff and demonstrated ability to provide clinical supervision.
