Permanent full-time, 80hrs/fortnight, includes ADOs, 5 weeks leave & salary packaging About the role Join Barwon Health’s Subacute at Home Program as an Allied Health Assistant and make a meaningful difference by supporting patients to achieve greater independence and wellbeing in their own homes.

Working across the Geriatric Evaluation and Management (GEM) and Rehabilitation streams, you will provide high-quality, patient-focused care under the direction and supervision of Allied Health Professionals. You’ll work as part of a multidisciplinary team, supporting patients through their rehabilitation journey while contributing to their goals and clinical outcomes.

This is a great opportunity for an experienced Allied Health Assistant who enjoys working with people, values collaborative care and is looking to broaden their skills within a dynamic community-based service.

Key responsibilities Provide patient-focused Allied Health interventions in patients’ homes under the direction of an Allied Health Professional.

Support patients across the GEM and Rehabilitation streams to achieve their health, wellbeing and independence goals.

Manage an allocated caseload and work collaboratively with Allied Health Professionals and the broader multidisciplinary team.

Complete clinical documentation and other administrative tasks accurately and within required timeframes.





What we're looking for Certificate IV or above in Allied Health Assistance, or an equivalent qualification.

Demonstrated experience and skills appropriate to working as an Allied Health Assistant in a clinical setting.

Experience supporting patients with exercise, rehabilitation or functional activities is highly desirable.

Strong communication and interpersonal skills, with the ability to build positive relationships with patients, families and colleagues.

Ability to work independently while also contributing effectively to a multidisciplinary team.
