Join Our Team as a Care Coordinator – Make a Difference in Your Community
We are seeking a dedicated and experienced Care Coordinator to oversee a portfolio of home care clients across the community. In this pivotal role, you will use your expertise in care coordination to ensure clients receive the best possible care and support, helping them maintain their independence and quality of life at home.
To be successful in this role, you will have:
- Experience in the Home and Community Care sector, with a solid understanding of Care Coordination and its associated administrative processes.
- Robust knowledge of Support at Home Package Client onboarding, funding, budgets, referrals, reviews, and associated administrative duties.
- Proven ability to manage care coordination efficiently, meeting the diverse needs of clients.
- Expertise in coordinating and managing client assessments, reviews, referrals, and maintaining thorough tracking and reporting systems.
- Exceptional organisational, prioritisation, and time management skills.
- Excellent problem-solving capabilities.
- A strong commitment to continuous improvement in service delivery.
- A compassionate, calm, and caring approach, with a high level of client-focused customer service.
- Experience working with vulnerable, ageing individuals, particularly those living with dementia or other health conditions.
- A positive, proactive, and engaging work ethic, underpinned by honesty and integrity.
- Ability to work independently as well as collaboratively within a team.
Your qualifications and registrations will include:
- At least 2 years of experience in Home Care Administration/Care Coordination.
- A current First Aid Certificate.
- A valid National Police Check (within the last 6 months) or willingness to obtain one.
- Availability to attend mandatory training and team meetings as required.
- A valid driver’s license and access to a reliable, registered, and comprehensively insured vehicle.
Your knowledge base will include:
- A thorough understanding of Aged Care Quality Standards and Support at Home requirements.
- Familiarity with continuous improvement principles in care services.
- Commitment to maintaining client privacy, dignity, and confidentiality.
- Awareness of workplace safety for both you and your clients.
- Experience working with culturally diverse populations and delivering culturally specific services.
- Dedication to working within a framework aligned with our mission and values.
