Description: We are seeking an exceptional Care Coordinator who has a passion for helping others, a robust commitment to client care and the ability to work collaboratively with a team of healthcare professionals to deliver the best possible outcomes.
Key Responsibilities: As our Care Coordinator, you will: Understand the Commonwealth Home Support program and HACC programs
Conduct assessments: including identifying any health risks, medical conditions, or social and environmental factors
Develop care plans
Coordinate care
Provide support and education.
Skills and Experiences: What we're looking for: Current Australian driver's licence
National police check
Working with vulnerable people/children check
