At Ozcare, we're committed to helping older Australians live independently and confidently in their own homes. We are currently seeking a compassionate and customer-focused Client Liaison Officer to join our Sunshine Coast Aged Care at Home team in a permanent part-time capacity.
As the primary point of contact for many of our clients and their families, you will play a vital role in ensuring older Australians receive the services, support and care they need to continue living well at home.
About the Role
Reporting to the Care Partnering Lead, you will be responsible for coordinating client care and providing exceptional service throughout the client journey. You will support recent and existing clients through intake, assessment, care planning, service coordination and ongoing reviews, helping to deliver a seamless and positive care experience.
Key Responsibilities
- Conduct client intake conversations, telephone assessments and service enquiries
- Assist with care planning, reviews and ongoing evaluation of client needs
- Coordinate and schedule services to support continuity of care
- Act as the primary contact for client, family and representative enquiries
- Build and maintain strong relationships with clients, families and internal stakeholders
- Monitor service delivery and identify changing client needs or risks
- Maintain accurate records, documentation and system updates
- Support onboarding processes for new clients
- Contribute to quality improvement initiatives and service excellence
- Ensure compliance with organisational policies, aged care standards and program requirements
About You You are a caring and organised professional who enjoys building relationships and making a positive difference in people's lives. You thrive in a fast-paced environment, demonstrate strong attention to detail and are passionate about delivering excellent customer service.
To be successful in this role, you will have:
- Experience in customer service,
administration, community services or aged care
- Strong interpersonal, verbal and written communication skills
- The ability to build rapport and establish trust with a diverse range of clients
- Excellent organisational and time management skills
- Strong computer literacy and data entry skills
- The ability to work both independently and collaboratively within a team
- Sound problem-solving skills and a proactive approach
- An understanding of community-based care services and referral pathways will be highly regarded

Requirements
- Current National Police Check (with validity in the past three years)
- Strong computer and mobile device skills
- Vaccination against Influenza and COVID-19 is strongly recommended
- Candidates with prior experience in a similar or identical role are highly desirable
