FYI Aged Care Services is a newly established Support At Home Provider committed to delivering expectational, person - centered care that enables older Australians to remain safe, independent in their own home.

As we begin this exciting journey, we are looking for a passionate and experienced Clinical Care Manager who is eager to be part of building something special. About the Role

As our Clinical Care Manager, you will be an important member of our leadership team, providing clinical knowledge and guidance to support the safe delivery of our services. Working closely with the CEO, Corporate Services Manager, Executive Management Team and support staff, you will help ensure our clinical practices align with the requirements of the Aged Care Act 2024, Aged Care Rules 2025, the Support at Home Program, the Strengthened Aged Care Quality Standards and professional nursing standards.

Together, we will build strong clinical systems, promote continuous improvement and ensure every consumer receives safe, high-quality, person-centered care.

What You'll Be Doing

In this role, you will work collaboratively with the leadership team to:

- Provide clinical guidance and support across our home care services.




- Assist with clinical assessments, reviews and care planning for consumers with varying support needs.
- Contribute to the development and maintenance of our clinical governance framework.
- Support the delivery of safe, evidence-based and person-centred care.
- Work with care staff to promote best practice, build clinical capability and provide mentoring when required.
- Participate in quality improvement activities, clinical audits and service reviews.
- Assist with incident reviews, investigations and continuous improvement initiatives, including SIRS requirements where applicable.
- Support medication management, infection prevention and control, falls prevention and other clinical programs.
- Build positive relationships with consumers, families, General Practitioners, allied health professionals and other healthcare providers.
- Contribute to the development and review of policies, procedures and clinical documentation.
- Help ensure services remain compliant with legislative and regulatory requirements.
- Encourage a positive workplace culture where teamwork, respect and learning are valued.
- Participate in organisational planning as we continue to grow our services.

About You





You are a Registered Nurse who enjoys working as part of a team and is passionate about delivering quality aged care.

You will bring

- Current unrestricted AHPRA registration as a Registered Nurse.
- Experience within aged care, community nursing or home care.
- Strong clinical assessment and care planning skills.
- Knowledge of the Aged Care Act 2024, Aged Care Rules 2025, Support at Home Program and Strengthened Aged Care Quality Standards, and a willingness to continue developing your knowledge.
- Excellent communication and relationship-building skills.
- A collaborative approach to leadership and problem solving.
- A genuine commitment to supporting older people to live safely and independently at home.
- Current National Police Check.
- Current Driver's Licence.
- First Aid and CPR.
- References to be provided during interview process
