Hospital is seeking energetic and innovative clinicians to lead our Post Operative Discharge Support Service within Surgical Services & Intensive Care. In this rewarding role, you will provide expert clinical leadership, coordinate postoperative discharge planning, and support patients transitioning safely from hospital to home. Working collaboratively with multidisciplinary teams, you will help ensure high-quality, patient-centred care that reduces avoidable readmissions and enhances the patient experience.

Your opportunity:

- The Clinical Nurse Consultant (CNC) is a clinical leader, accountable at an advanced practice level for the coordination of patient centred clinical practice specific to Post Operative Discharge Support Service (PODSS).
- The CNC demonstrates sound knowledge of contemporary nursing practice and theory and provides clinical leadership, advice and advocacy to a multidisciplinary team and is responsible for the facilitation and provision of quality and cost effective nursing and/or midwifery services which are consistent with contemporary standards, guidelines, relevant legislation and best practice.
- Maintain a close relationship with the Nursing Director Surgical Services and Intensive Care, Clinical Directors, key stakeholders and community services.
- The CNC will report to the Nursing Director Surgical Services and Intensive Care.

Permanent Full Time (76 hours p.f.) or Permanent Part Time Positions (hours negotiable).

Qualifications and other requirements:





- Mandatory possession of a nursing qualification from a registered tertiary institution or completion of an approved hospital training program as a Registered Nurse and Registration with the Australian Health Practitioner Regulation Agency with a current annual practicing certificate is essential.
- Appointment to this position requires proof of qualification and if applicable registration or membership with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
- Possession of context-specific postgraduate qualification is highly desirable.

Our offer: You will receive a competitive remuneration package value up to $174,208 p.a., comprising salary between $140,631 - $152,684 p.a. (f/t)



or salary rates between $70.92 - $77.00 p.h.
