Revira Care is seeking compassionate, reliable, and experienced Disability Support Workers to join our team in Oberon, NSW.

We have an urgent requirement for a Support Worker to provide community access and person-centred support to an NDIS participant. This is an excellent opportunity for someone who is passionate about supporting participants to maintain independence, engage with their community, and achieve their individual goals.

About the Role

As a Disability Support Worker, you will provide community access and daily living support based on the participant's individual needs and NDIS goals.

Your responsibilities may include

- Supporting the participant to access the local community and attend appointments, activities, and outings.
- Assisting with shopping, errands, recreational activities, and social participation.
- Supporting the participant to build confidence and independence in the community.
- Providing companionship and encouraging meaningful community engagement.
- Following the participant's individual support plan and preferences.
- Maintaining the participant's dignity, privacy, choice, and independence.
- Providing safe and respectful person-centred support.
- Completing accurate progress notes and reporting any incidents or concerns appropriately.
- Working collaboratively with the Revira Care team and other relevant stakeholders.

- Good at communication and active listening.
- Reliable and punctual with a professional attitude.
- Able to work independently while following support plans and organisational procedures.
- Available to commence immediately.

Essential Requirements
- Previous experience as a Disability Support Worker or in a similar community support role.
- NDIS Worker Screening Check.
- Current First Aid and CPR Certificate.
- National Police Check.
- Australian working rights.
- Current Australian Driver's Licence.
- Reliable vehicle with appropriate insurance.
- Relevant disability/community services qualification is desirable.
