Flow Healthcare is looking for an experienced Occupational Therapist to join our growing community team in a permanent part-time position, working 20 hours per week.

Your role will involve supporting older adults to live safely and independently in their homes, as well as supporting adults living with disability through the NDIS.

This is a rewarding opportunity suited to an OT who is reliable, practical and enjoys providing high-quality clinical work in a supportive environment. The role is structured to provide appropriate paid time for client visits, travel, documentation, report writing, liaison and other non-face-to-face clinical work.

About the role

- Permanent part-time – 20 hours per week

- Paid time for travel, documentation, report writing and clinical liaison
- Community-based work supporting older adults accessing the Support at Home program and adult NDIS participants
- Work locations include clients' homes, residential aged care facilities (RACF) and community settings
- Flow Healthcare is a fully mobile healthcare service, with documentation and non-face-to-face tasks completed remotely/from home
- Consistent referral flow and scheduling support
- Structured face-to-face induction, shadowing and ongoing clinical support

What you'll be doing

- Provide in-home OT services to Support at Home clients with diverse health needs
- Provide community-based OT services to NDIS participants




- Conduct assessments and implement goal-directed, evidence-based interventions
- Complete functional assessments and reports
- Prescribe equipment including wheelchairs, assistive aids and minor home modifications
- Provide falls and pressure injury prevention, education and interventions
- Provide manual handling assessments, education and intervention
- Prepare clear, timely and clinically sound reports
- Collaborate with clients, care managers, support coordinators, health professionals and other stakeholders
- Complete documentation and administrative tasks within expected timeframes
- Work in line with professional standards, organisational processes and relevant NDIS and aged care requirements
- Communicate effectively with clients, families, staff and other health professionals

What we're looking for

- Degree in Occupational Therapy
- Current AHPRA registration as an Occupational Therapist with no restrictions
- Minimum 2 years' experience in community-based occupational therapy practice
- Experience and competence completing functional assessments and reports, including initial assessments, falls assessments, functional capacity assessments and assistive technology assessments
- Ability to work independently within established clinical and operational expectations
- Strong clinical reasoning and practical problem-solving skills
- Strong communication, organisation and time management skills




- Valid Australian driver's licence and access to a reliable vehicle with comprehensive insurance
- Current Police Check and NDIS Worker Screening Check, or willingness to obtain
- Current First Aid and CPR
- Willingness to obtain annual influenza vaccination
- Full working rights in Australia
