Position Overview The Outreach Case Manager provides specialist community-based case management and outreach support to women and children who have experienced domestic and family violence. The role supports women to improve safety and wellbeing in their home environment through intake, risk and safety assessment, safety planning, case management, practical and emotional support, referral, advocacy and access to accurate information.
The position is based across Dawn House locations in Darwin and provides outreach in clients’ homes and community settings as required. The role may also be required to work from other Dawn House locations to meet operational needs.
The role operates under the general direction of the Team Leader and is responsible for the following main areas:
· Provide trauma-informed, strengths-based, feminist and culturally responsive outreach case management to women and children experiencing domestic and family violence.
· Conduct in-home risk and safety assessments, develop collaborative safety and case plans, and coordinate practical measures that support women and children to remain safely in their homes and sustain tenancies where appropriate.
· Coordinate referrals, advocacy and service linkages with relevant government and non-government agencies to respond to clients’ immediate and longer-term needs.
· Maintain high-quality case records, SHIP data and reporting, and meet child safety, confidentiality, privacy, professional conduct and safe work requirements, including participation in after-hours arrangements as required.
Essential Criteria
Knowledge, Skills and Experience:
· Demonstrated experience in domestic and family violence, outreach, homelessness, community services or a related human services setting, including individual case management and client support.
· Sound understanding of domestic and family violence dynamics, trauma, risk escalation, safety planning and the impacts of violence on women and children.
· Demonstrated ability to apply trauma-informed, strengths-based, feminist, culturally safe and client-centered practice with women and children experiencing domestic and family violence.
· Demonstrated ability to undertake intake, assessment, case planning, safety planning, practical support, advocacy and referral within organisational procedures and qualified supervision.
· Demonstrated risk assessment skills, including the ability to identify changing risk, manage competing priorities, respond appropriately to crisis and escalate concerns within delegation.
· Sound knowledge of child safety and child protection responsibilities, mandatory reporting obligations, confidentiality, privacy, professional boundaries and relevant Northern Territory service systems.
· Ability to deliver outreach and in-home support safely, work with appropriate autonomy in community settings, and follow approved procedures for lone work, client contact and risk management.
· Ability to support women to strengthen safety and tenancy stability, including coordinating in-home safety measures or alternative accommodation responses where required.
· Strong written communication, case recording, data quality and reporting skills, including accurate use of SHIP or other client management systems and completion of required monthly reports.
· Sound stakeholder engagement skills, including the ability to make timely referrals, build effective service linkages and advocate respectfully with government and non-government agencies on behalf of clients.
· Hold a Northern Territory Police Clearance and Working with Children Clearance (Ochre Card) or be able to obtain and maintain the required clearances.
· Current Northern Territory Driver License, or the ability to obtain one, and capacity to participate in after-hours or on-call arrangements in accordance with service requirements.

Qualifications:
(Essential)
· Relevant degree in Social Work, Human Services, Social Science or a related discipline; or
· Diploma, Certificate III or Certificate IV in Community Services, or another relevant qualification, with demonstrated experience and competence sufficient to perform the duties of a Dawn House Level 4 position; or
· An equivalent combination of previous appointments, service, study, experience and expertise sufficient to meet the requirements of the role.
(Desirable)
· Specialist training in domestic and family violence, trauma-informed practice, risk assessment, safety planning, outreach case management or related fields.
Desirable Criteria:
· Experience working in a specialist domestic and family violence service, homelessness service, women’s service, outreach program or comparable non-government community services environment.
· Experience providing community-based or in-home support that contributes to client safety, housing stability or tenancy sustainment.
· Experience working with Aboriginal and Torres Strait Islander women and children, culturally and linguistically diverse communities, and services operating in the Northern Territory context.
Key Duties
Outreach case management
Maintain an allocated caseload and provide responsive outreach case management for women and children experiencing domestic and family violence, including intake, assessment, case planning, practical and emotional support, advocacy, referral and information within delegated authority.
Risk, safety and in-home support
Conduct in-home risk and safety assessments and work collaboratively with clients to develop and maintain current safety plans. Coordinate appropriate in-home safety measures and, where required, support access to alternative accommodation or other immediate safety responses.
Case planning and tenancy sustainment
Develop and review client-led case plans that identify immediate and longer-term needs, monitor progress and support women and children to improve safety, wellbeing and housing stability, including sustaining tenancies where appropriate.
Child safeguarding and mandatory reporting
Identify and respond to child safety or child protection concerns, document and escalate matters promptly, and comply with mandatory reporting obligations, relevant legislation and Dawn House procedures.
Referral, stakeholder engagement and advocacy
Facilitate timely referrals and maintain effective linkages with relevant government and non-government agencies. Advocate for clients within case-specific and delegated authority and contribute to coordinated service responses.
Records, SHIP data and reporting
Maintain accurate and timely client records in SHIP, including assessments, case plans, risk and safety assessments, referrals and case notes. Complete required documentation and monthly reporting within organisational timeframes and support accurate, complete client data.
Teamwork, supervision and service coordination
Participate actively in case review, staff meetings, supervision and team activities; communicate emerging risks, workload issues and service barriers to the Team Leader; and contribute to consistent, coordinated and high-quality outreach service delivery.
Organisational contribution
Comply with Dawn House policies, procedures, privacy, confidentiality, WHS and skilled conduct requirements; participate in continuous improvement and professional development; participate in after-hours arrangements as required; and undertake other duties consistent with the Level 4 classification and role scope.
