Dignicare Health Services Pty Ltd is a privately owned Western Australian company based in Perth.
We offer NDIS, Support At Home, DVA and CHCP support services to clients within WA.
Our team consists of experienced medical professionals and community service workers who offer services to enable clients to live their lives with more independence and ease.
We are currently seeking a registered nurse to join us in a part-time or casual capacity with the possibility of going full-time.
In your role, you will be part of a multidisciplinary team providing continuity of care in a home setting, ensuring dignity, safety, and respect at all times.
Please note the detailed list of responsibilities and requirements for this role below:
- Conducting accurate, comprehensive assessment of clients presenting without appointments to improve outcomes and minimise adverse events
- Assess and develop chronic disease management care plans that direct health care interventions and activities by others
- Identify and provide comprehensive physical, psychosocial and emotional care for patients
- Provide high-level care assessments and advise of implications for the care team
- Proactively supporting clients’ independence and social connections while promoting positive health & wellbeing.
- Checking, preparing, and administering all medications in accordance with state legislation and organisational policies and procedures
- Providing care and support to clients in line with their personalized care plans.
- Injections, application, and removal of casts
- Diagnostic services including ECG, spirometry, stress tests, urine drug screening, and hearing tests.
- Communicate the company’s policies, procedures and processes to all stakeholders
- Educate personnel, including support workers and carers,
on how to support clients with complex health needs including performing complex health care procedures
- Take reasonable care for your own health and safety
- Report any incident or hazards to the relevant manager or supervisor.
- Keep informed of the contents of the NDIS and aged care systems and apply them to daily work.
Competencies and experience
- Experience in a similar role for at least 1-2 years
- Being a part of a multi-disciplinary team delivering improved health outcomes to our clients
- Good people skills are a must, as this job is all about working very closely with participants
- High level of attention to detail & the ability to multitask
- Experience in writing assessments and reports to monitor participants' progress
- Ability to work in team and maintain a good relationship; also ability to work unsupervised
- Dedication to delivering high-quality evidence-based services
- Demonstrated effective interpersonal, verbal, and written communication skills.
- A commitment to excellence in nursing practice and continuing professional development.
- Compassion and enthusiasm, with a friendly, approachable attitude
Requirements:
- Current AHPRA registration as an registered nurse
- Experience in suprapubic catheter care/changes.
- Demonstrated competence in manual handling, hoist use, and diabetes care.
- Police clearance check (current and valid).
- First Aid & CPR certification
-Valid driver's license -Current working rights in Australia
Job Types: Part-time, Subcontract, Casual
Pay: $55.00 – $65.00 per hour
