About the role An exciting opportunity exists to join our Community Mental Health Service, working across the Flexible Assertive Community Treatment (FACT), Acute Continuing Care Team (ACCT), and Older Person's Mental Health Service (OPMHS). This role provides an opportunity to deliver person-centred, recovery-focused care to consumers experiencing mental health challenges within the community.

The successful applicant will be responsible for monitoring consumers' mental state, contributing to mental health assessments, and supporting the development and implementation of care plans. Working closely with consumers, carers, families, prescribers, and other healthcare professionals, you will play an important role in promoting positive mental health outcomes and supporting individuals throughout their recovery journey.

Strong assessment and observation skills are essential, with the ability to identify changes in presentation, assess risk, and respond appropriately to emerging clinical needs.

The role requires excellent communication skills and the ability to engage with consumers in a respectful, compassionate, and non-judgemental manner while maintaining effective relationships with multidisciplinary team members and external stakeholders.

Applicants must demonstrate an ability to remain calm and professional in complex or emotionally charged situations and utilise effective crisis intervention and de-escalation strategies when required.





You will join a highly skilled multidisciplinary team comprising Social Workers, Psychologists, Health Clinicians, Registered Nurses, a Transition Nurse Practitioner, and Clinical Nurse Consultants. Together, the team provides comprehensive assessment, treatment, support, and care coordination services to individuals across a range of community mental health settings.

This position involves rostered shifts of 8:30am to 5:00pm and 8:00am to 8:30pm, including weekend work, across the FACT, ACCT, and OPMHS teams.
