Working in close partnerships within the business you will lead an integrated, mobile and multidisciplinary clinical service that connects specialist and home care nursing, allied health practitioners and external clinical partners across Warrigal's Residential Homes, Home Services and Village settings, enabling older people to feel known, loved and connected.

In this role you will build sustainable in-house expertise for rising acuity, earlier intervention, subacute support and hospital avoidance while developing eligible nursing and allied health alternate funding streams, improving workforce and service coordination and strengthening clinical partnerships.

In this role you will

- Integrate a service strategy and model design that is responsive to resident and client needs, and aged care reform requirements
- Establish end-to-end access and workflow for clear referral and escalation criteria, transparent triage processes, responsive scheduling, fair prioritisation and reliable matching of the right specialist resource.
- Create positive working relationships within the organisation operations partners
- Promote early intervention without creating inappropriate barriers through specialist nursing and hospital avoidance.
- Partner with Home Services and Villages operations supporting connection across care contexts.
- Lead an allied health model that focusses on health promotion and combines risk-based assessment and intervention
- Lead organisation-level relationships setting service expectations and performance measures with external clinical partnerships and contracts
- Participate in governance forums and turn learning into revised pathways and resource decisions.
- Ensure financial sustainability with maximisation of funding streams and targets supporting clinical need, rights, clinical safety and professional judgement.

The requirements | What it takes to do this work

- Tertiary qualification in nursing, allied health or another relevant health profession, with current qualified registration where the profession is registered.
- Substantial senior leadership experience in complex community, aged care or integrated clinical services, including accountability for multidisciplinary practitioners.
- Demonstrated experience leading geographically dispersed or mobile services, including demand management, scheduling, triage, workforce allocation and service redesign.
- Strong understanding of care of older people, rising acuity, hospital avoidance, transition care, chronic and complex care, palliative care, dementia and mental health.
- Demonstrated commercial and financial capability, including budgets, contracts, service costing, funding or revenue streams and performance reporting in a clinical environment.
- Current unrestricted drivers license and capacity to travel regularly across the organisation.
