Berry Street is seeking a Senior Practitioner – Complex Care Services to deliver high quality support and case management to young people in our TCP and LT programs in the northern metro region of Melbourne. The role is based at Preston with a hybrid in office and work-from-home arrangement.
You will lead a small team, mentor practitioners, develop individual case plans, and work closely with the Team Leader on program development, stakeholder management and reporting.
