Right at Home Gosford is seeking a qualified and experienced ClinicalCare Partner - EN/RN to help clients live empowered and independent lives at home. Our mission is to improve the quality of life for those we serve.
You will coordinate client needs within their funding model, conduct care assessments and create care plans, train care workers, ensure documentation is complete in Right at Home databases, and provide some community nursing care as required.
