We are looking for a compassionate and experienced Support Coordinator who enjoys building solid relationships, solving complex problems and advocating for positive outcomes.
This role will suit someone who thrives on variety, enjoys working autonomously while being supported by an experienced team, and wants to make a meaningful impact every day.
Based in Bunbury, you will manage your own portfolio of participants while working closely with families, guardians, therapists, service providers and the NDIA to coordinate high-quality supports that place participants at the centre of every decision.
What You'll Be Doing
Every day is different, but your responsibilities will include:
Empowering participants to understand and implement their NDIS plans
Working alongside participants to achieve meaningful goals and build greater independence
Helping participants make the most of their NDIS funding through effective planning and budget monitoring
Connecting participants with quality service providers and community supports
Coordinating complex supports and facilitating team-oriented outcomes between providers, families and stakeholders
Advocating for participants to ensure their voices are heard and their choices are respected
Supporting participants through plan reassessments, changes in circumstances and challenging life transitions
Preparing high-quality documentation, including case notes, reports and funding evidence
Building trusted, long-term relationships with participants and their support networks
