As a Credentialled Diabetes Educator, you'll play a vital role in supporting individuals living with diabetes through education, clinical management and ongoing care. Working as part of a multidisciplinary team, you'll provide evidence-based diabetes education, promote self-management, and support improved health outcomes for patients across a variety of healthcare settings.
Your responsibilities may include:
Providing diabetes education and self-management support for people with Type 1, Type 2 and gestational diabetes.
Conducting comprehensive assessments and developing individualised care plans.
Supporting patients with insulin initiation and adjustment, glucose monitoring and diabetes technologies including CGM and insulin pumps.
Delivering education to patients, families and healthcare specialists.
Working collaboratively within multidisciplinary teams.
Participating in health promotion and chronic disease management programs.
Maintaining accurate clinical documentation and adhering to best practice guidelines.
About You
To be successful, you will have:
Current unrestricted AHPRA registration as a Registered Nurse.
Credentialled Diabetes Educator (CDE) status.
Postgraduate qualification in Diabetes Education and Management.
Current accreditation with the Australian Diabetes Educators Association (ADEA).
Experience managing Type 1, Type 2 and gestational diabetes.
Experience using Continuous Glucose Monitoring (CGM) and insulin pump technologies.
Current Basic Life Support (BLS) certificate.
Excellent communication and interpersonal skills.
Ability to work independently and collaboratively within multidisciplinary teams.
