Aspire4Life is seeking a Clinical Aged Care Assessor to join our team across Gippsland (Gippsland region, covering Traralgon and Bairnsdale and surrounds). You will undertake comprehensive aged care assessments in clients’ homes, helping them access services and maintain independence.
You’ll develop holistic care plans, identify supports and assistive technologies, and work with clients, families, and healthcare specialists.
