ForHealth Group's Bondi Medical Centre is seeking an experienced Part time Chronic Disease Management (CDM) Nurse to join the team. You will work with GPs and patients to support the prevention and management of chronic conditions, empowering patients to take control of their health.
In this role you will coordinate GPMPs and TCAs, conduct health assessments, educate patients, and collaborate with multidisciplinary staff to promote proactive care and better patient outcomes.
