About Us

The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers.
Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care.
Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as

An exciting opportunity has become available to join the NBMLHD Sustaining NSW Families team and make a meaningful difference in the lives of local families.




This service provides home-based antenatal and postnatal support for families across the Penrith and Hawkesbury Local Government Areas who need extra assistance during their child's first two years of life.
As a Registered Nurse, you will work collaboratively to deliver safe, compassionate, and person-centred care alongside patients, families, carers, and the multidisciplinary team.
This role offers the opportunity to support families in their homes, build strong relationships, and contribute to positive health and developmental outcomes for children.

What you will bring to the role
Current registration as a Registered Nurse Division 1 with AHPRA.
Demonstrated extensive, recent experience and clinical skill in the provision of care to children 0-2 years and their families/carers as a registered nurse including completion of a new graduate RN program and willingness to complete the Advanced Nurse Practice course within 12 months of employment
Demonstrated capacity to provide effective nursing care that is person centred, family inclusive and culturally sensitive to people accessing health services.
Demonstrated ability to organise and prioritise work.
Demonstrated effective communications written and verbal skills.
Demonstrated commitment to professional development
Ability to work effectively within a multi-disciplinary team.
Current Drivers Licence valid within Australia and willingness to use for work purposes.
Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa.
