GNA Services is seeking a compassionate, reliable and experienced
Home Care Support Worker
to provide person-centred support to elderly clients living in their own home in Kalgoorlie, WA.
The successful candidate will assist clients to maintain their independence, safety, wellbeing and community participation while respecting their individual goals, preferences and dignity.
About the Role
This is a
part time/casual opportunity
supporting an elderly couple with a range of home care and community support needs.
Your responsibilities may include:
Domestic assistance including vacuuming, mopping, bathroom and toilet cleaning, general household cleaning, laundry and linen.
Shopping assistance and running errands.
Transport and accompaniment to appointments and community activities.
Social support, companionship and community access.
Supporting clients with mobility while following safe manual handling practices.
Monitoring client wellbeing and reporting any changes to the Care Coordinator.
Supporting clients to maintain independence and achieve their care plan goals.
Completing accurate progress notes and incident reports.
Following GNA Services policies, aged care standards and workplace health and safety requirements.
About the Clients
The role involves supporting clients with a range of support needs, including:
Acquired brain injury
Stroke-related disability
Reduced mobility and wheelchair dependence
Chronic pain and limited endurance
Cardiovascular conditions
COPD
Diabetes
Depression




High falls risk
Experience supporting people with complex or multiple support needs will be highly regarded.
Essential Requirements
Applicants should have:
Certificate III in Individual Support (Ageing, Disability or Community), or willingness to obtain.
Experience in aged care, home care, disability or community services.
Current National Police Clearance.
Current First Aid and CPR Certificate.
Valid Australian driver's licence.
Reliable and appropriately insured vehicle.
Excellent communication and interpersonal skills.
Ability to work independently in clients' homes.
Understanding of person-centred care, dignity and client choice.
NDIS Worker Screening Check
is desirable.
Desirable Experience
Experience with the following would be advantageous:
Stroke or neurological conditions
Acquired brain injury
Mobility limitations
Mental health support
Aged Care Home Care Packages
Falls prevention
Safe manual handling
Supporting elderly couples
