Salary Range: $75,000 to $85,000 plus superannuation

POSITION OVERVIEW
We are seeking a compassionate and experienced Care Supervisor to lead our direct care team across aged care and disability support services.
This is a full-time, ongoing supervisory role shall be responsible for overseeing the planning, coordination and delivery of high-quality, person-centred care services to older people and individuals with complex support needs and to supervise, guide and support a team of aged care workers, disability support workers and personal care workers, while ensuring all services are delivered in compliance with legislative, ethical and quality standards.
KEY RESPONSIBILITIES:
Service Coordination & Delivery
· Coordinate, monitor and review individualised care and support services for older people and NDIS participants, ensuring services are responsive to changing client needs and aligned with their goals
· Facilitate the development, implementation and ongoing review of individual service plans in collaboration with clients, families and multidisciplinary teams
· Coordinate client-directed services, supporting consumer choice and control in all aspects of service delivery
· Deliver and oversee care services using a palliative approach,
providing dignified end-of-life care in accordance with best practice standards
Supervision & Workforce Leadership
· Supervise aged care workers, disability support workers and personal care workers on a day-to-day basis, providing clear direction, mentoring and performance guidance
· Conduct regular team briefings, support debriefs and individual supervision sessions to maintain workforce wellbeing and service quality
· Identify training and development needs within the team and provide or coordinate relevant training, coaching and support
· Promote independence and community participation among staff and clients alike
Risk Management & Complex Needs
· Identify, assess and implement targeted interventions for older people at risk, including those experiencing social isolation, functional decline or complex health presentations
· Escalate concerns appropriately and contribute to risk management planning in line with organisational policy and legislative obligations
Compliance & Governance
· Manage legal and ethical compliance across all service activities, including adherence to the Aged Care Quality Standards, NDIS Practice Standards and relevant South Australian legislation
· Maintain accurate client records, incident reports and service documentation in accordance with privacy and confidentiality obligations
· Ensure all staff operate within their scope of practice and comply with relevant certifications and compliance requirements (Police Check,
NDIS Worker Screening, etc.)
Partnerships & Networks
· Develop and maintain professional networks and collaborative partnerships with health services, community organisations, allied health professionals and government agencies
· Manage and review partnership agreements with contracted service providers, ensuring obligations are met and service quality is upheld
· Represent Care Squad in stakeholder meetings, case conferences and community forums as required
TO BE SUCCESSFUL IN THIS ROLE YOU WILL REQUIRE:
· AQF Certificate IV in Individual support or higher relevant qualification is required.
· 12 months full-time or equivalent part-time or casual basis employment with proven experience is required in relevant field.
· Demonstrated experience supervising or leading a team in a care setting is highly desirable
· Valid Australian driver's license is must, along with comprehensive car insurance.
· Applicants must be willing to accept shift allocations at client locations across metropolitan, regional, and outer regional South Australia, depending on operational requirements and client care needs.
Flexibility to travel between client locations is an essential requirement of this position.
· Strong interpersonal skills, a compassionate, patient approach.
· Ability to work independently and as part of team.
· A valid National Police Check, Working with Children Check and other NDIS entry level certificates are required.
· First Aid, Manual Handling and CPR Certificate is required.
· Sound understanding of the aged care and/or NDIS legislative frameworks
